2021 – Flu Vaccination Programme

Appointments are available on the 20th of October from 13:30-18:00 at Smallthorne Community Centre. If you are unable to attend this day there will be alternative clinics in the future. Bradeley Village residents will be offered the vaccination on an alternative date at Bradeley Village.

2021 Flu Vaccination Appointment Booking Form
Appointments are only available if you are over 50 years of age and/or are in an at risk group as shown below:

• Those aged 6 months to under 50 years in clinical risk groups
• chronic (long-term) respiratory disease, such as
asthma which requires continuous or repeated
use of inhaled or systemic steroids or with
previous exacerbations requiring hospital
admission, chronic obstructive pulmonary disease
(COPD) or bronchitis
• Chronic heart disease, such as heart failure
• Chronic kidney disease at stage 3, 4 or 5
• Chronic liver disease
• Chronic neurological disease, such as Parkinson’s
disease or motor neurone disease
• Learning disability
• Diabetes
• Splenic dysfunction or asplenia
• A weakened immune system due to disease (such
as HIV/AIDS) or treatment (such as cancer
• Morbidly obese (defined as BMI of 40 and above)

You are also eligible if you are:

• Pregnant
• Aged 50 years and over
• In a long-stay residential care homes
• A carers
• A close contact of an immunocompromised
• Frontline health and social care staff employed by:
a registered residential care or nursing home,
registered domiciliary care provider, a voluntary
managed hospice provider or a Direct Payment
(personal budgets) and/or Personal Health
Budgets, such as Personal Assistants.

Appointments are on Wednesday 20th October from 13:30 until 18:00 at Smallthorne Community Centre. There is indoor queuing space if needed and full disabled access. Please select your preferred appointment time below and we will confirm your appointment either by phone or text message. Please wear a mask if you are able to do so as well as loose clothing on your arms.
